MONARCH CROWN AWARDS: OFFICIAL AWARD RULES

OVERVIEW

The Monarch Crown Awards ("Award") recognizes exceptional board-certified OB-GYNs who demonstrate outstanding patient care, professional excellence, and community impact.

SPONSOR

The Award is sponsored by Monarch MD, Inc. ("Monarch").

ELIGIBILITY

  • Nominees must be board-certified OB-GYNs actively practicing in the United States at the time of nomination.
  • Nominees must have a minimum of ten ( 10 ) years of post-residency experience.
  • Nominations may be submitted by patients, colleagues, or mentees over the age of 18. Self-nominations are accepted.
  • Employees, contractors, or affiliates of Monarch or their family members are not eligible to be nominated.
  • By submitting a nomination, nominators confirm that they and their nominees meet all eligibility requirements.

NOMINATION PERIOD

The Award nominations open on September 9, 2025 and close on October 27, 2025, at 11:59 PM Eastern Time ("Nomination Period").

NOMINATION PROCESS

  • Nominations must be submitted through the official Monarch Crown Awards online nomination form available at https://monarchmdinsights.typeform.com/crownawards during the Nomination Period.
  • Each nomination must include:
    • Nominator's full name
    • Nominator's relationship to the nominee
    • Nominator's valid contact information (email and phone number)
    • Nominee's full name and practice information
    • Written description of the nominee's qualifications based on the award criteria
  • Incomplete, inaccurate, or late submissions will not be considered.
  • There is no limit to the number of nominations a person may submit, provided each nomination is for a different nominee.
  • There is no fee to enter.

JUDGING & SELECTION

  • All eligible nominations will be reviewed by a selection committee designated by Monarch.
  • Nominations will be evaluated using a weighted rubric that considers:
    • Impact on others (30%)
    • Alignment with Monarch's values (20%)
    • Strength of story/nomination depth (25%)
    • Community recognition/peer validation (15%)
    • Potential for storytelling and amplification (10%)
  • The shortlist of finalists will be announced the week of October 27, 2025.
  • Winners in two categories, Excellence in Patient Care and Excellence in Mentorship, will be announced the week of November 3, 2025.
  • All decisions of the selection committee are final.

RECOGNITION & PRIZES

  • Each winner will receive:
    • Public recognition through Monarch's channels and press outreach
    • A $3,000 Four Seasons e-gift card (travel and additional expenses not included)
  • Prizes are non-transferable and not redeemable for cash.
  • Approximate retail value (ARV) of each prize package: $3,000.
  • Winners are responsible for all federal, state, and local taxes associated with prize acceptance.

WINNER NOTIFICATION

  • Potential winners will be notified via email and/or phone using the contact information provided during the nomination process.
  • If a potential winner cannot be contacted within five ( 5 ) business days of the first notification attempt, or if a potential winner declines a prize, Monarch may select an alternate winner.
  • Winners may be required to complete an affidavit of eligibility, liability release, and publicity release (where legal) within a specified timeframe to claim their prize.

PUBLICITY & RIGHTS

  • By participating, nominators and nominees grant Monarch the right to use their names, likenesses, photographs, voices, and biographical information for advertising, publicity, and promotional purposes in any media without additional compensation, unless prohibited by law.
  • Nominees selected as winners agree to participate in reasonable publicity activities, including interviews and promotional events.

PRIVACY

  • All information submitted in connection with the Award will be treated in accordance with these Official Rules and Monarch's Privacy Policy, available at monarchmd.com/privacy.
  • By participating, nominators and nominees consent to the collection, use, and disclosure of their personal information as described in these Official Rules and Monarch's Privacy Policy.

GENERAL CONDITIONS

  • Monarch reserves the right to cancel, suspend, or modify the Award if fraud, technical failures, or any other factor beyond Monarch's reasonable control impairs the integrity of the Award, as determined by Monarch in its sole discretion.
  • Monarch reserves the right to disqualify any individual it finds to be tampering with the nomination process or the operation of the Award or to be acting in violation of these Official Rules.
  • Any attempt to deliberately damage any website or undermine the legitimate operation of the Award may be a violation of criminal and civil laws. Should such an attempt be made, Monarch reserves the right to seek damages to the fullest extent permitted by law.

LIMITATIONS OF LIABILITY

  • By participating, nominators and nominees agree to release and hold harmless Monarch and its respective affiliates, subsidiaries, advertising and promotion agencies, and each of their respective officers, directors, employees, and agents from any liability, injury, loss, or damage of any kind arising from or in connection with the Award or any prize won.
  • Monarch is not responsible for: ( 1 ) any incorrect or inaccurate information; ( 2 ) technical failures of any kind; ( 3 ) unauthorized human intervention in any part of the Award; ( 4 ) technical or human error that may occur in the administration of the Award or the processing of nominations; or ( 5 ) any injury or damage to persons or property that may be caused, directly or indirectly, in whole or in part, from participation in the Award.

DISPUTES

  • Except where prohibited, participants agree that: ( 1 ) any and all disputes, claims, and causes of action arising out of or connected with the Award or any prize awarded shall be resolved individually, without resort to any form of class action, and exclusively by the appropriate court located in Dover, Delaware; ( 2 ) any and all claims, judgments, and awards shall be limited to actual out-of-pocket costs incurred, but in no event attorneys' fees; and ( 3 ) participants waive all rights to claim punitive, incidental, and consequential damages and any other damages, other than for actual out-of-pocket expenses, and any and all rights to have damages multiplied or otherwise increased.
  • All issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules shall be governed by and construed in accordance with the laws of Delaware without giving effect to any choice of law or conflict of law rules.
